APPLY FOR GRADUATION

THIS IS THE OFFICIAL PROCESS NECESSARY FOR YOU TO APPLY TO GRADUATE AT THE END OF THE 2013 FALL SEMESTER OR THE 2014 WINTER SEMESTER.

Please follow the instructions below to apply for graduation:

1.  Follow these links to download and print the check sheet(s) for your major(s), two copies of the Courses in Progress form and the University Requirements check sheet.   If you are an HRIM or HPIS major, please follow this link to your Courses in Progress form.  Check your catalog year to determine which version of the check sheet you will complete. For example, if you entered UD in the fall 2008, you would complete the check sheet that corresponds to fall 2008.
2.  Complete all forms in black or blue ink.
3.  Submit one completed copy of each of the courses in progress form, the major check sheet(s), and the University Requirements check sheet to your faculty advisor(s)  for his/her signature by Friday, March 8th. You should keep one completed copy of the courses in progress form for your records. Your faculty advisor(s) are listed below.
